Twitterでの著作権侵害申し立ての記入方法についての個人的なまとめです。これが正式な解説である保証はありません。和訳の不備などが含まれる可能性があります。あらかじめご了承ください。

確実に連絡の取れるEメールアドレスを記入する。※フリーメールアドレスは避けた方がいいという解説もあり。（http://alpharalpha.x.fc2.com/parts/uploader_tuhou.txt より）
Street address:
申し立て人の在住町・番地の記入欄。
City:
申し立て人の在住市の記入欄。
State/Province:
申し立て人の在住都道府県の記入欄。
Postal code:
申し立て人の在住郵便番号の記入欄。
Country:
申し立て人の在住国の記入欄。
Phone number:
(optional)
申し立て人の電話番号の記入欄。（オプションのため記入は必須ではない。）日本の国番号は「81」のため、「+81.」をつけた市外局番の最初の0を抜いた番号を記入する。※電話番号が「0123456789」の場合は「+81.123456789」となる。
